Please fill print and fill out the listed forms. Once complete, you can email the completed form to westsidebassmasters@yahoo.com. Registration fee may be paid in cash, check, or through Venmo (@Ryan-Fontenot-05). Once the registration forms are submitted along with payment, you will be added to the roster for the current year.
